1. Agreement to Terms

By accessing the website at https://sharonphysiotherapy.ca, booking appointments, or receiving treatment at Sharon Physiotherapy, you agree to be bound by these Terms of Service, all applicable laws and regulations, and agree that you are responsible for compliance with any applicable local laws.

2. Clinical Services & Appointments

• Scheduling: Appointments are booked online via Noterro or by phone. A valid health history form must be completed prior to your first session.
• Cancellation Policy: We require at least 24 hours' notice for any cancellations or modifications to your appointment. Late cancellations or missed appointments without proper notice may be subject to a fee.
• No-Shows: Failure to attend a scheduled session without contacting the clinic will result in a full or partial charge of the session cost.

3. Fees and Payments

• Payment Responsibility: Patients are fully responsible for all fees associated with their assessment and treatment.
• Insurance Billing: We offer direct billing to select extended health insurance providers as a courtesy. If direct billing fails, is rejected, or only partially covers the cost, the outstanding balance must be settled immediately by the patient.
• MVA & WSIB: We accept claim coverage for motor vehicle accidents and workplace injuries. Patients must provide valid claim numbers, coordinator info, and accident details prior to direct billing.

4. Conduct and Safety

We maintain a safe, respectful environment for both our clinical staff and patients. Sharon Physiotherapy reserves the right to refuse service, terminate care, or request individuals to leave the premises in cases of harassment, discrimination, or abusive behaviour.
